Make a copy of the lease or purchase contract from an authorized U.S. BMW dealer
Make a copy of your valid U.S. driver's license
Obtain a doctor's note or permit
Request a copy of invoice detailing the modifications performed
Email the above documents to BMWMobility@bmwusa.com or fax to 201-930-8484

All qualified modifications should be completed within BMW's New Limited Vehicle Warranty or CPO Warranty programs. Claims may take up to four weeks to be processed.
